誤會大了！耳鼻喉科不是只看感冒！

路人甲一副裝熟的樣子提高音調說：

「好久不見！聽說你是耳鼻喉科醫師，那你『一定』很會看感冒囉！」

醫師乙三條線地回答:

「我不是只有看感冒！就算看感冒，也不是只看大人！兒童腸病毒也會看！」

感冒要看哪一科？內科、家醫科？還是耳鼻喉科？一般民眾似乎比較喜歡找耳鼻喉科醫師，一個很重要的原因就是耳鼻喉科醫師有很多「雞絲(台語)」，可以幫病患做局部治療。比如說，如果感冒鼻涕很多，耳鼻喉科醫師會利用抽吸器把鼻子裡面的膿鼻涕抽掉，病人頓時覺得鼻子通暢多了。但反過來說，一般人找耳鼻喉科醫師看病的主要病因就是感冒，因而一般耳鼻喉科門診的病患大宗也就變成感冒病人，尤其冬天時可能有超過一半的病人來源都是因為感冒來就醫的。久而久之，一般人會有一種錯覺，以為耳鼻喉科就是看感冒的「小科」。其實這是一般人對耳鼻喉科醫師的誤解，而且誤解可大了！筆者身為耳鼻喉科醫師，有必要為一般民眾就筆者所認識的耳鼻喉科做些介紹。有興趣的、想知道為什麼的朋友，請繼續往下看！

「第12屆台日耳鼻喉頭頸外科研討會」和「第95屆耳鼻喉科全國醫學會」在上週順利落幕。這次醫學會除了瞭解日本耳鼻喉科專家目前的最新發展外，也更清楚國內各家醫院耳鼻喉科的發展現況。筆者發現國內各家醫院耳鼻喉科都相當努力發展自己的特色，甚至不斷突破與創新！例如北榮蕭安穗部主任的「耳硬化症微創手術」、中榮和台大皆在發展頭頸癌的「達文西機器手臂微創手術」等，族繁不及備載！除了各家醫學中心的學術及技術發展，更令人感動的是許多基層診所的前輩也都紛紛回來聆聽演講，更新自己的臨床知識，真種終身學習的進取精神真是後生晚輩值得效法學習的。

看到耳鼻喉科的先拜如此盡心盡力地為醫學發展付出奉獻，筆者不禁要為同業發聲：我們真的不是只有看感冒！就算看感冒，也不是只看大人，兒童腸病毒也會看！一般民眾對耳鼻喉科醫師存在著很大的誤解，其實耳鼻喉科是個內(內科)、外(外科)兼修的大科，在國內許多醫學中心的編制中，耳鼻喉「科」甚至升格成耳鼻喉「部」，下分耳科、鼻科及喉科。

早期耳鼻喉科本屬於外科體系，後來由於分科逐漸精進發展，外科部編制開始改變，某些專業科別獨立出來而脫離外科部。以北榮為例，1958年國內成立台北榮民總醫院，當時耳鼻喉部原本只是外科部的一個科。1975年正式脫離外科部，成為獨立科。1984年耳鼻喉科正式升格為部，並設耳科、鼻(頭頸)科及喉(頭頸)科。

耳鼻喉科因為涵蓋疾病眾多，因而細分三科。後來更因每位醫師在各自領域有所專精，甚至再細分為內、外科疾病。以耳科為例，有所謂耳內科(例如治療耳鳴等)、耳外科(例如珍珠瘤手術等)的區別。因為耳鼻喉科這種可內(內科疾病)可外(外科疾病)、進可攻(開大刀)退可守(基層診所)的獨特性質，讓耳鼻喉科受到許多年輕醫師的青睞。有人就開玩笑地說：「所謂四大科內外婦『ㄦ』，這個『ㄦ』就快要從二聲的『兒』變成三聲的『耳』了！」為了防止內外婦兒四大科變成「四大皆空」，行政院衛生署把全國教學醫院每年住院醫師的訓練容量分配做了很大的調整。就耳鼻喉科而言，訓練員額已經被腰斬，從以往每年可以訓練將近80人的訓額變成目前的40多人。以往教學醫院只要3個主治醫師就可以招收1位住院醫師，現在要5位主治醫師才能招收1位，標準變的非常嚴格。以2013年為例，耳鼻喉科專科醫師訓練醫院容額只剩49名(衛署醫字第102270129號函)。

言歸正傳，要成為一位耳鼻喉科專科醫師必須經過5年既辛苦又紮實的訓練。住院醫師所要學習的範圍上到顱底，下到鎖骨(圖7)。這個範圍所涵蓋的手術琳瑯滿目，小到耳科的中耳通氣管手術、鼻科的鼻中膈彎曲矯正手術、喉科的頸部小腫塊切除手術，大到耳鼻喉三科的惡性腫瘤(外耳道癌、鼻咽癌、鼻竇癌、口腔癌、喉癌、下咽癌甚至顱底手術等)切除及重建手術。尤其癌症術後的照護及追蹤更是治療的重點，耳鼻喉科醫師需要隨時評估病患術後是否出現復發情形。術後病患的生活品質、全喉切除後的食道語或人工發生器的使用訓練，也都是耳鼻喉科醫師照護病患的重點。

在國外，耳鼻喉科醫師還負責顏面整型及重建手術。在台灣，衛生署於2013年將「耳鼻喉科訓練課程基準」修正為「耳鼻喉科範圍之顏面整形重建手術」(衛署醫字第 1020271444號函)。目前正夯的「變臉手術(face transplant)」是個極為困難的手術(影片1)，需要多團隊的合作，其中耳鼻喉科醫師就扮演了舉足輕重的角色，因為耳鼻喉科醫師對於顏面部的解剖構造再清楚不過了。當把別人的臉接到另一個人的臉時，哪一條血管要接在哪裡，可一點都馬虎不得，一旦接錯致使血流不過，新接上的臉就會壞死，到時不是想換一張臉就可以換，可能活不活的下來都是一個問題。

此外，國外也針對喉部創傷或全喉切除的病人發展出「喉部移植重建手術(larynx transplant，影片2)」。以往接受全喉切除的病人，要不使用氣動式或電子式人工發聲器(artificiall larynx)，但聲音聽起來非常不自然，像單調的機器人聲音；要不學習食道語(esophageal speech)，但非常不易學習，很多病患最後都放棄，改用人工發聲器；雖然有醫師透過手術在氣管-食道壁之間穿洞，把氣體從肺部導入食道咽部交接處來發音(tracheoesophageal speech)，發聲品質較食道語更自然，但仍然不夠理想，尤其病患每次講話都要堵住頸部洞口以防漏氣，甚為不便。不論以上述何種方式發聲，都不方便也不自然。現在「喉部移植重建手術(larynx transplant)」的出現，對病人而言是一大福音，而這也有賴耳鼻喉科醫師對喉部及周圍相關解剖構造的瞭解才能完成。

再者，鼻整型也是耳鼻喉科醫師的強項。一般人都以為只有整型外科才會做鼻整型，其實耳鼻喉科對鼻部的解剖構造再清楚不過。國內在三軍總醫院耳鼻喉部的努力下，於2005年創立了「台灣顏面整形重建外科醫學會」，並經常舉辦研習會和手術教學訓練，同時經當和韓國的鼻整型界舉辦交流研討會，與世界各國顏面重建整形醫學會維持良好的互動，甚至成為顏面整形外科醫學會國際聯盟International Federation of Facial Plastic Surgery Societies (IFFPSS)的成員之一，對國內鼻整型領域發展著實貢獻不少！

再來談談兒童感冒到底要看哪一科吧。兒童感冒或腸病毒屬於小兒常見疾病，所有醫師都應有診斷及治療的能力，並須能夠分辨重症個案，及時轉診。在耳鼻喉科領域，「小兒耳鼻喉科學」是耳鼻喉專科醫師基本訓練，耳鼻喉科專科醫師考試指定教科書Bailey's Head and Neck Surgery: Otolaryngology就把「小兒耳鼻喉科學」獨立出來成為一個專區(section)，總共涵蓋22個章節(圖10)。在專科醫師考試當中，「小兒耳鼻喉科學」也是每年必考的重點項目之一。

而且，上呼吸道是耳鼻喉科醫師每天都在看的區塊，不論是用反射鏡或內視鏡，因此對上呼吸道的解剖構造和病變相當熟悉，對良性或惡性病灶的判斷上也更為精準，對疾病的輕重緩急更是拿捏得宜。尤其兒童常見感冒合併積液性中耳炎，在判斷中耳是否積水，耳鼻喉科醫師就有絕對的優勢。以筆者為例，經過台北榮總耳鼻喉部5年紮實的訓練，以耳鏡看過的耳朵保守估計就超過5萬隻，這還不包含用內視鏡看的案例。此外，目前醫院的耳鼻喉科門診及大多數的耳鼻喉科基層診所都配備有內視鏡，大幅改善兒童中耳積水的診斷率。

但是別小看耳部內視鏡的操作，要知道兒童的耳道極為窄小，加上兒童常常躁動不配合，如果不熟悉內視鏡操作，一不小心就會刺傷外耳道而鮮血直冒，甚至可能刺破耳膜，造成醫源性傷害，站在一旁的父母鐵定被嚇死！另外，一旦中耳積水未能及時診斷出來，對兒童語言發展或上課學習會有相當負面的影響，嚴重的案例甚至會導致耳膜塌陷、慢性中耳炎或膽脂瘤的併發症。兒童小小的感冒，卻可能演變成後續需要開刀的嚴重後遺症，可說得不償失。

最後，針對急性中耳炎併發中耳積水，在還沒有達到放中耳通氣管條件下，可以利用「雷射耳膜造口術(laser myringotomy，圖11)」來消除中耳積水，立即改善兒童聽力。同時對於吃了很久的抗生素但中耳炎一直不見改善的個案，「雷射耳膜造口術」還可以讓耳鼻喉科醫師把中耳內的積水送去作細菌培養化驗，以瞭解到底是哪一種細菌這麼毒？竟然一般抗生素都殺不死！「雷射耳膜造口術」這種精密的雷射手術需要耳鼻喉科醫師的專業技能方能既迅速又安全地完成，不然對於不配合的小朋友，光是要把雷射槍對準耳膜安全部位，可能就要耗上一段時間，還不見得打的到。

圖11.「雷射耳膜造口術」是個精密的雷射手術，一般初學者可能耗了半天還無法把雷射槍對準耳膜安全部位。即使打到了，也可能因為角度拿捏不好，導致耳膜一直打不破，把病人折騰地半死。

當然，筆者必須強調的是，兒童感冒不是一定只能看耳鼻喉科，或非耳鼻喉科不可，筆者想說的是耳鼻喉科醫師在上呼吸道感染的診治方面，專業能力是無庸置疑的！

筆者才疏學淺，在耳鼻喉科這個領域的資歷尚淺，對耳鼻喉科領域參與的廣度和深度都遠不及先拜前輩，因此本文內容或有缺漏，亦不免有所謬誤，敬請大家見諒！此外，筆者對於許多耳鼻喉科醫師正在研究或發展的新技術也因為瞭解不夠，不敢妄自介紹說明。惟希望透過本文，讓一般民眾瞭解耳鼻喉科醫師「會看感冒」，但「不只看感冒」！

About

Dr. Chin-Lung Kuo was the Director of Dr. Kuo ENT Clinic. He used to be the directot of the Department of Otolaryngology, Taoyuan Armed Forces General Hospital, Taiwan. Currently, he is the deputy editor of Archives of Otorhinolaryngology-Head and Neck Surgery. Dr. Kuo received his medical degree from the National Defense Medical Center and earned his PhD degree at the Institute of Brain Science, National Yang-Ming University, Taiwan. He completed his otolaryngology residency, chief residency, and fellowship training at the Department of Otolaryngology-Head and Neck Surgery, Taipei Veterans General Hospital, Taiwan.

Dr. Kuo continues to make contributions to the scientific community. His primary field of study is otology, and his research is focused on auditory neuroplasticity. Dr. Kuo has been the first or corresponding author on more than 70 published articles, including two book chapters, SCI original papers, and reviews.

In addition to research, Dr. Kuo has also devoted a great deal of time to education. He used to be the the staff of the National Yang-Ming University School of Medicine and the National Defense Medical Center, Taiwan. In 2013, the National Yang-Ming University School of Medicine awarded him the Faculty Excellence Award in Teaching.

In 2012, his research paper was chosen as the cover story of the journal Audiology and Neurotology. In addition, Dr. Kuo received the Outstanding Paper Award from the Laser Medicine Education and Research Foundation in 2013 and was awarded the 2015 Hayashi SPIO (Society for Promotion of International Otorhinolaryngology) Scholarship in Japan.

In 2021, the prestigious Stanford University has recently published an update of the list of the top 2% most widely cited scientists in different disciplines, the World's Top 2% Scientists. This ranking, considered the most prestigious worldwide, is based on the bibliometric information contained in the Scopus database and includes more than 190,063 researchers from the more than 8 million scientists considered to be active worldwide, with 22 scientific fields and 176 subfields taken into account. The list recognized Dr. Kuo as one of the top 2% scientists that has brought great pride to Dr. Kuo and his research team.

Dr. Kuo believes that increased availability of research data can benefit both medical research and science education. As deputy editor, he is delighted to lead Archives of Otorhinolaryngology-Head and Neck Surgery, a journal that provides a new avenue for researchers to publish their research and advance the field of otorhinolaryngology.
